How to get contracted with Molina Healthcare of California as a doula

Molina Healthcare of California is a Medi-Cal plan in several regions, including the Inland Empire, where it’s the commercial plan alongside IEHP.   1. Enroll with Medi-Cal and get an NPI

    Molina points doulas to DHCS enrollment first: enroll as a Medi-Cal provider and get an NPI (see how to become a Medi-Cal doula). Its resources page notes doula claims can come from an individual doula, an enrolled doula group, or an enrolled licensed provider that employs or contracts with a qualified doula.

  2. Work through Molina’s credentialing checklist

    Molina’s Doula Credentialing Requirements Checklist (updated February 2026) is short:

    • A current W-9, dated within the past 12 months.
    • A business license, not required if you operate in a rural area.
    • Your training pathway documentation or doula certificate.
    • Any limitations on your services (for example age range, populations served, or postpartum only).
    • Insurance: professional/general and umbrella liability. Ask Molina for the limits.
    • A completed Non-Clinical Provider Form.
    • A fax number, which Molina uses to send contracting documents and letters of agreement.
    • The doula taxonomy code 374J00000X on your NPI.
  3. Request a full contract (or start under an LOA)

    Full contract: contact Molina’s Contracting Department; Molina’s toolkit points to its doula contact list, and DHCS lists Maria Torres (maria.torres6@molinahealthcare.com) for doula contracting.

    Letter of Agreement (LOA): when a member asks for a doula in an area where Molina hasn’t finished contracting, a non-contracted doula can request an LOA. It’s triggered by a prior authorization or continuity-of-care request: Molina’s utilization management team decides whether an LOA is needed and passes it to contracting, which confirms rates and signs it. An LOA covers one member under that authorization, and Molina keeps working on your full contract in parallel.

    Molina’s doula training (March 2026) says doulas aren’t contracted until the contract is executed and loaded into Molina’s system, that credentialing starts then, and that credentialing can take 45 to 90 days. Return documents promptly to avoid delays.

  4. Bill through SSI or on red-and-white CMS-1500s

    • Electronic: through the SSI Claimsnet (SSI Group) clearinghouse, payer ID 38333.
    • Paper: only on original red-and-white CMS-1500 forms, typed in 10- or 12-point Times New Roman in black ink. Copies, black-and-white forms and handwriting are rejected. Mail to Molina Healthcare of California, P.O. Box 22702, Long Beach, CA 90801.
    • Bill with the Molina member ID.
    • Status and authorizations: Availity Essentials. Authorization requests for visits beyond the standing recommendation must include the licensed provider’s renewed recommendation.
    • Disputes: Molina Healthcare of California, Attn: Provider Dispute Resolution Unit, P.O. Box 22722, Long Beach, CA 90801, or through Availity. Molina responds in writing within 45 working days.
