For doulas in Health Net, CalViva and Imperial Valley counties
How to get contracted with Health Net, CalViva Health or CHPIV as a doula
Health Net Community Solutions, CalViva Health (Fresno, Kings and Madera counties) and Community Health Plan of Imperial Valley use the same doula workflow: a Doula Provider Participation Application, reviewed in cycles. Health Net’s current application also lets doulas apply for its commercial plans, not just Medi-Cal. Here’s the process for all three.

Check Health Net’s four eligibility questions
Health Net’s workflow starts with four yes-or-no questions. You can apply if you answer yes to all of them:
- Experience supporting pregnant people and their families before, during and after birth.
- Availability to offer in-person services in your community.
- You meet DHCS’s minimum requirements for doulas.
- You’re registered in PAVE, Medi-Cal’s enrollment system. (The application form lets you mark your PAVE application as in process.) See how to become a Medi-Cal doula.
Submit the right plan’s Doula Provider Participation Application
Download the application for the plan you’re joining (Health Net, CalViva Health, Community Health Plan of Imperial Valley) and email it to DoulaSupport@healthnet.com. Health Net’s November 2025 form asks for:
- Whether you’re a doula or doula collective, and the lines of business: Medi-Cal and/or commercial (HMO, PPO, POS).
- Your name or DBA, tax ID, NPI, addresses, contact details, website and social handles, and how you want to receive member referrals.
- Experience with the Birth Equity populations of focus (Black, American Indian and Alaska Native, Pacific Islander), your doula experience, and your links to local organizations.
- How soon you can start serving members (0–60, 60–90 or 90–120 days).
- Copies of your business license and certifications, and your PAVE status.
- Certificates of insurance: general liability and professional liability, each at least $1,000,000 per occurrence and $3,000,000 aggregate (or self-funded insurance details).
- The counties you want to serve, with the number of active doulas and whether services are in person or virtual in each.
Wait for the review cycle
You’ll get an email confirming receipt and that your application is on the list for the next review cycle. Health Net decides based on your eligibility, its network needs (it wants doulas to have enough volume to sustain their programs) and your ability to serve in person in the counties you requested.
Applying isn’t a guaranteed contract, but Health Net keeps applications on file and re-reviews them as network needs change. Contracted doulas expanding into new counties fill in a shorter section, and Health Net looks at their existing referrals and claims.
Serve members: eligibility, documentation, recommendations
- Eligibility: confirm the member is active for the month of service in Health Net’s provider portal (preferred) or the state’s AEVS line, 800-456-2387.
- Documentation: Health Net publishes a Documentation of Doula Visit form for each plan; any format that meets DHCS’s documentation rules is accepted.
- Recommendations: DHCS’s standing recommendation covers the initial package. For up to nine more postpartum visits, you can help the member get a recommendation from any licensed practitioner, on Health Net’s or DHCS’s form, and keep it in the file.
Bill by claim or invoice
- EDI or Availity Essentials (preferred).
- Invoice: contact your assigned point of contact.
- Paper CMS-1500 (02/12): Health Net – CalAIM Invoice, P.O. Box 10439, Van Nuys, CA 91410-0439, or fax 833-386-1043. Paper takes longer.
- No authorization number is needed on doula claims.

- How do I become a doula provider with Health Net?
- Make sure you meet DHCS’s doula requirements, can serve in person and are registered in PAVE, then email the Doula Provider Participation Application for Health Net, CalViva Health or Community Health Plan of Imperial Valley to DoulaSupport@healthnet.com.
